While there have been some improvements in both state Medicaid program transparency and access since 2014, most states still have discriminatory restrictions that keep patients from being cured. Here are the restrictions that are the most significant barriers to HCV treatment:
Requiring prescribers to obtain advance approval before insurance will cover HCV treatment
Based on a patient’s degree of liver damage or fibrosis level
Based on a patient’s drug or alcohol use
Based on a prescriber’s specialty or expertise
Based on whether a patient has previously received treatment
Assesses parity between a state’s fee-for-service program and contracted managed care organizations
Restrictions that are less common but may pose severe barriers to care
*States with an asterisk next to their grade have removed prior authorization for most patients entirely

As of February 2024, 28 states have removed prior authorization for first-time treatment: AK, AZ, CA, CO, CT, DC, DE, FL, HI, ID, IL, IN, KS, LA, MA, MI, MO, NC, NH, NY, OK, OR, PA, RI, TX, VA, WA, WI.
